2024 was a defining year for Access Afya — a year that marked strategic growth, operational evolution, and a renewed focus on creating sustainable, community-based healthcare solutions.
Staying true to our mission of developing sustainable healthcare models for the global mass market, we continued to expand our digitally enabled franchise platform that supports primary healthcare workers to deliver high-quality, affordable, and comprehensive care to low-income families across Kenya.
As we transitioned toward a fully franchised model, our structure became leaner and more specialized — focused on franchisor support rather than directly operating clinics. This shift allowed us to scale efficiently while maintaining high standards of quality and sustainability.
Expanding Reach Across Kenya
Access Afya’s network grew significantly in 2024, expanding from 13 to 20 locations — now serving 20 low-income communities across Kenya. Our reach now covers an estimated 400,000 people, up from 260,000 in 2023.
Much of this expansion occurred toward the end of the year, with strategic entry into peri-urban and rural areas to complement our established urban presence. As these sites mature, we anticipate even greater community impact in 2025.
Our Curafa franchise model continues to be a cornerstone of this growth — providing local entrepreneurs with the tools, technology, and operational guidance to deliver care closer to where people live and work.
Empowering Health Workers and Strengthening Systems
In 2024, Access Afya developed, trained, or retained 100 healthcare workers in under-resourced communities — up from 65 in 2023. Through ongoing training and access to digital tools, these health workers are better equipped to deliver accurate diagnoses, timely follow-ups, and improved health outcomes.
Our proprietary digital platform, AfyaSmart, is now used by hundreds of health workers, enabling real-time data capture, guided care delivery, and performance tracking across the network.
This technology-driven approach ensures consistency in care quality and empowers healthcare workers to operate efficiently while improving patient experiences.
Delivering Quality Care and Strong Outcomes
Access Afya remains committed to delivering care that is both affordable and high quality. Key health outcomes and operational indicators for 2024 include:
Patient Satisfaction (Net Promoter Score): 85 (up from 79 in 2023)
Patient Self-Reported Recovery: 92% (vs. 93% in 2023)
AfyaScore – Quality Scorecard: 93% (unchanged from 2023)
Virtual Clinic Visits: 18,242 (up from 17,333 in 2023)
Total Health Visits: 98,020
By combining physical clinics with digital healthcare, Access Afya continues to make care accessible and effective for families across different settings.
Sustainability in Action
Environmental responsibility is an integral part of how we operate. As a tech-enabled healthcare organization, we’re committed to reducing our carbon footprint and managing resources responsibly.
In 2024:
31% of sites were powered by solar or hybrid clean energy (up from 25% in 2023)
33 devices were refurbished (up from 11 in 2023)
15 units of e-waste were recycled
These efforts reflect our ongoing commitment to sustainability, extending the lifecycle of our devices and ensuring our operations remain environmentally conscious as we scale.

Our franchise network thrives because of the passion and commitment of local healthcare entrepreneurs who bring essential services to their communities.
“I was motivated to join the franchise after seeing people travel long distances to access medical services. I wanted to bring quality, affordable, and accessible healthcare closer to the Mugutha community,” shares Purity Kibaara, Curafa Medical Clinic – Mugutha. “The franchise has been instrumental in my clinic’s growth — from helping identify the right location and providing setup support, to offering flexible credit and ongoing training.”

Access Afya’s model continues to demonstrate operational sustainability while maintaining affordability for patients and partners.
Total Network Revenue Generated: KES 20,019,275 (2023: KES 24,773,840)
Average Revenue Per End User: KES 264 (2023: KES 296)
Average Direct Cost Per End User: KES 70 (2023: KES 96)
Average Indirect Cost Per End User: KES 815 (2023: KES 769)
Tax, Licenses, and Levies Paid: KES 17,298,733 (2023: KES 12,097,735)
While total revenue saw a slight decline due to structural changes, our transition to a fully franchised model has positioned us for scalable, long-term growth. Costs are expected to decline over time as our network expands and efficiencies increase.
Looking Ahead: Scaling Impact and Broadening Services
As we move into 2025, Access Afya is focused on scaling sustainably and deepening impact. Following a successful pilot, we plan to expand access to mental health services within our primary care model — addressing a growing community need.
We’ll continue to strengthen our franchise ecosystem, improve data systems, and forge new partnerships that support universal health coverage.
